EXPLORing NEW TECHNOLOGY PLATFORMS AND SYSTEMS OF CARE FOR PERFORMING MINIMALLY INVASIVE SURGICAL PROCEDURES



Traditional Operating Rooms are inefficient and overcrowded. Patient data are not integrated and displayed to caregivers in a timely fashion, and turnover time between cases is lengthy.

Technologies designed to impact procedural medicine are often introduced in isolation, usually failing to improve efficiency and safety, or reduce costs. Devices are often haphazardly introduced into a technologically complex environment. Integrating high technology components, however, is not sufficient to achieve the goal of better patient care; teamwork and communication in a high tech environment is equally essential.

To address many of these problems, CIMIT pioneered the MGH “Operating Room of the Future” (ORF) project. The ORF is a living laboratory that explores new technology platforms and systems of care for performing minimally invasive surgical procedures. Accurate data capture and analysis, multidisciplinary teamwork, and thoughtful integration of technology are the building blocks in this environment that optimizes patient safety and comfort, staff satisfaction, and financial efficiency.

The ORF is a testament to the power of collaboration. Launched in the Massachusetts General Hospital (MGH) in August of 2002, as a fully functional OR for minimally invasive procedures as well as for open surgery. It represents an extraordinary convergence of disciplines – clinicians, institutions, and private companies – all brought together by CIMIT, and major financial and intellectual investments by all parties. Advanced by critical partnerships, a number of pioneering programs are currently being explored in the ORF:

  • Real-time workflow process improvement including parallel processing, novel IT architecture, asset management and patient tracking solutions;
  • Clinical decision support systems combining patient-specific data with intelligent devices to create a perioperative zone of safety;
  • Leading a national initiative on technology integration and open Plug-and-Play standards for medical devices; and
  • Measuring outcomes to inform “evidence-based” OR facility design.

With this living, working, flexible learning environment, CIMT and MGH clinicians have been altering surgical care delivery. Beyond the ORF per se, procedures and workflow techniques developed here have propagated to the conventional ORs and have improved them through education, as well as by raising expectations of what is possible and by evaluating and choosing the best of technologies. Through our research, we will understand factors for optimal facility design, technology requirements, and deliver specifications for optimal team design and training. Additionally, we expect to show reduced medical errors and improved patient comfort and post discharge follow-up.

The active nature and beat of the expansion of new technologies that is foreseen in the coming years are challenging the surgeons and other decision makers. Hospitals are often stimulated to make decisions before there is definitive evidence on their efficacy and impact.

  3. Nowadays, modern technological innovations are rapidly developing.I really admire with the new the technologies other nations used in their hospitals like in the USA and Japan. No wonder why most of the Filipinos who can afford chooe to seek medical help outside the country. What makes me sad is the fact that our own country which is the Phil. seems to be left behind of the rapidly developing technologies of the other countries.

    Government Hospitals did not gave much efficiency and quality care unlike with the private one. It's sad thinking most Filipinos can't afford so most of us chose to seek medical help at the government hospitals.

    New technologies must be introduced to the hospitals here in our country in order to reduced the morbidity and mortality rate here. It would be a great help to the health care providers since it will reduced hassles and medical errors as well as providing comfort and quality care to the clients.

It's a big help for the doctor's and nurses the new technology.it has the big impact for us Filipinos but we don't have here in the Philippines the hi-technology.Especially in OR it will decrease the mortality. and it will not hard on the part of the doctors.and in wards it is also important.an in doing some procedures it will help us to lessen our work.
    Like before some neurosurgeons its hard for them/difficult to remove o tumor,it's like a game, that you have to decide if you will play or not.but in the past decades,we have our imaging system that provide clearer pictures of the brain structure,so in that matter its a big help for them to know what the problem really is.it a big thing for us also 'cos we will also understand why this happen.but we need to have some hi-tech in the Philippines.that's why sometimes we are thankful for new technologies.
    We need here in the Philippines so that our co-Filipinos will not go abroad to do there operation there.it will help Filipinos life.

  6. This article catches my attention.It seems very interesting topic.It has able to answer my questions in my mind. As a student nurse, we were trained to and have exposed to special areas either operating room or delivery room. After reading this article, I appreciated it and able to compare the settings of the operating room Where i was exposed a day ago.
    The OR setting was not properly managed because there were instruments that are not arranged in proper cubicle.And then, when the doctorswere having a sugical procedures, the area was overcrowded, and so, there would be a possibility that the are would become unsterile which may lead infection to patient. It is because microorganisms easily enter the open wounds or broken skin since the doctor is performing a surgical procedure. Another thing is that, some of the student nurses tackles about the condition of the patient wherein they have no right to spread the information. Especially when the patient is in critical condition. It would be stressful for the patient and to the family of the patient once they heard negative feedback. So, you are not comforting the patient anymore, you are worsening the life of the patient.
    So, once you read this article, this plan would be approprite in our OR's hospitals. For sure if this would be applied here in the Philippines, all the OR's in the hospitals would be efficient. The service would be excellent.
    We all know that technology makes the life of the filipinos easier. Especially in the hospitals, technologies contributes more and makes the work of the doctors' easier.
    But now, here is the question, "HOw could the administration able to applied this plan if our country is facing economical crisis? And, don't forget, our country is having crisis too, in terms of technology.

Interesting topic, i hope it would be implemented asap so that we "Filipino's" can experience on having this kind of devices & we can give or accommodate our patient a very comfortable, high tech procedure during surgery. Especially to our surgeon to have an easy & almost perfect procedure they can be. And most important the benefits of having this minimal invasive surgery. As what my sister told me that this kind of procedures involve a few or very small incisions when compared with open surgery like we did here. Good for our client because the smaller the incision, the smaller the scar they may & of course a very quick recovery. We are lucky to have this kind here in our country...

  8. Modern technologies greatly helps the surgeon and the OR nurses in the Operating Room because of its advantages. In the Philippine settings, the government can't afford with these technologies mainly because of corruption, poverty and many more problems . If these technologies were applied by the government, their would be much effective care provided to the clients .

Not only the patient be benefited in the said Technology but also the staff.
    But if we figure out the OR setting of today especially in government hospital there’s a lot of instruments and technology are lacking not because of the Filipino are delayed to discovered the newest technology although that is one of the reason also, but the main basis is that because of the financial problem, for we all know that we are facing economical crisis nowadays. I wonder why, why is it that the hospital can’t afford to buy newest technology? For we all know that government’s employee are have higher salary compare to the private employee. In addition there’s a lot of client admitted in government hospital but it is sad to know that they can’t provide the full interest to the said client.

  12. It is sad to know that the Operating room setting in this modern world specifically here in the Philippines is not as highly equipped and developed compared to other countries. Nowadays, the number of people soars high. What I'm trying to emphasize is that as the population grows, we need additional facilities to respond the needs of our patients in the operating room settings in particular. Based on the daily news, traditional Operating Rooms are inefficient and over crowded which is truly realistic.

    Aside from that, based on my experience in the actual setting there are some devices or equipment that need attention because these are over used and should need a replacement.

    These are only examples of evidences that our government must attend. Remember, one's health is very vital.

    Despite these concerns, CIMIT pioneered the MGH “Operating Room of the Future” (ORF) project which is a big help that mainly explores new technology platforms and systems of care for performing minimally invasive surgical procedures. Furthermore, procedures and workflow techniques developed here have propagated to the conventional ORs and have improved them through education, as well as by raising expectations of what is possible and by evaluating and choosing the best of technologies.

    I am hoping that all problems with regards to O.R. facilities will be solved gradually. May our government correspond to the needs of the health care providers as they render their services to the patients.     This article really interests me. This technology is truly a big help for the health care team and also the patients. The doctors will have a more effective viewing of the surgical sites through the monitor that they will be looking at therefore they won’t have to strain their heads while operating and because they have to use the specially designed surgical instruments for this kind of surgery which will be placed through the incision, it will be less hassle for nurses because they won’t have to hand each needed instruments all the time. This is also advantageous for the patients because they will have significantly less pain, less chance of infection, a shorter hospital stay and faster recovery.
    I just hope that this kind of technology will soon be available in our country so student nurses will have to experience it first here and we will have more knowledge about it rather than experiencing it later in abroad and also to provide the most efficient and effective services to our patients.

  19. Technology is a great influenced in many ways. Most of us are using technology to help us. It is much better that the technologies are more advanced so it has great advantage for us not only to monitor but it helps us make our job easier especially we are dealing with people's lives. Technology really conquered us since we cannot really make our job easier without it. So much help we can get with these technologies. But the disadvantage is if we don't know how to use it, then it is useless and might lead to bigger problems.

    It is just disappointing that Philippines is still needs improvement to make a great service to patients. I hope one day we can have those advanced technologies to improve our health care services.

  23. Having this "Operating Room for the Future" would definitely increase the quality of service being served to the people needing such medical services. The facility not only gives assurance to the patients to be healed but more, it brings confidence to the surgeons and the rest of the medical staffs performing the medical procedures. Technology definitely found its way to reach people through this facility.

    As for our country, we are too far to be able to have this kind of facility. Even some of the private medical facilities (hospital and other health care centers) cannot come at par with facilities like ORF. Having this type of facility available to many of the Filipinos would surely improve the quality of medical services that can be offered in our country. It might seem impossible for now, but if the right people will work on these, it would not be impossible after all. ORF will be an inspiration.

  24. As I read this article it comes up to my mind that advancement upon technology is necessary in order to attain an appropriate and much better way of rendering care. New discoveries may help the surgical team to prevent certain circumstances that may cause serious damage to the client and promote faster recovery. Regarding to my experience it really takes plenty of hours to finish a certain procedure especially for much invasive operations.

    So to speak having an advancement in the platforms of operational procedure will make it immediately done in a short period of time. As of now there are still scientific discoveries that still searching and developing the standard methods of operation. I'm still looking forward that it will come out someday and improve our skills as well as to make us more equip.

  34. Living in the 21st generation, it is only appropriate that crucial technologies should be developed especially those concerning health care. Development of operating rooms into a fully functional and standard operating rooms would be very efficient in rendering health care services.

    Once this would be realized, not only would it be efficient for the health care providers but it will greatly affect the morbidity and mortality cases here in our country.

    although our country is facing economical depression, i think that it is time for our government to take in hand the matters concerning the efficiency of the health care system, they should give consideration that the state of facilities and the health care provider's skills are important in maintaining quality health care services, not that we have problems with our medical practitioners for we have the best filipino doctor's and nurses.
